Trinity Health’s Saint Francis Hospital, Hoffman Heart & Vascular Institute, seeks an RN specializing in heart failure to deliver expert, evidence‑based nursing care for adults with acute and chronic heart failure. You will collaborate with a multidisciplinary team, educate patients and families, and monitor hemodynamics, weights, telemetry, and medications.

Night shift, part‑time, with every other weekend.

Qualifications

• Graduate of an accredited nursing program.
• Current RN licensure in Connecticut.
• BLS certification (AHA/ARC).

Responsabilités

• Deliver nursing care to patients with heart failure across disease stages.
• Perform detailed cardiac assessments and monitor hemodynamics, fluid status, weights, symptoms, and telemetry.
• Administer and evaluate heart failure medications using nurse-driven protocols.
• Collaborate with physicians and the care team to coordinate patient care.
• Educate patients and families on disease management and lifestyle changes.
• Recognize and respond promptly to changes to prevent deterioration.
