Job Title: Nurse Practitioner / Physician Assistant - Heart Failure Clinic


Location: Jackson, MI


Inpatient/Outpatient: 10% Inpatient, 90% Outpatient.


Case Load: 100% Adult



Position Overview:



Henry Ford Health Jackson is seeking a skilled and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our Heart Failure Clinic team. This full-time role will focus on providing comprehensive care to post-hospital discharged heart failure patients. The ideal candidate will have at least one year of experience working as an Advanced Practice Provider (APP) in cardiology, with a strong background in heart failure management. No new graduates will be considered for this position.

Key Responsibilities:



  • Provide expert care for heart failure patients post-hospital discharge, working closely with both employed and community physicians.

  • Collaborate effectively with primary cardiologists and hospitalists to build trusting relationships and encourage referrals to the heart failure clinic.

  • Ensure continuity of care and manage complex patient cases in an independent, fast-paced environment.

  • Focus on patient outcomes and delivering high-quality care to improve heart failure management.





Schedule - Monday - Friday, 5 days, 8-hour shifts.

GENERAL SUMMARY: The Specialty/Invasive Nurse Practitioner (NP) will work in a collaborative, multidisciplinary environment. The Specialty/Invasive NP will be responsible for the assessment, diagnosis, treatment, education, consultation, and care coordination as a first line provider to patients and their families. Provides inpatient and/or outpatient medical care. Works in collaboration with attending physicians, advanced practice providers, consultants, house officers, nurses, and other members of the medical care team to ensure that patient outcomes are achieved safely and efficiently. Specific procedures as per credentialing as indicated by training and experience. Works in one of the following departments: Cardiovascular Medicine, Emergency Medicine, ICU, Interventional Radiology, Nephrology, Neurology, Neurosurgery, Oncology, Orthopedics, Otolaryngology, Pulmonary and Critical Care Medicine, Surgery, Urgent Care, Urology. EDUCATION/EXPERIENCE REQUIRED: Master's degree in nursing. CERTIFICATIONS/LICENSURES REQUIRED: Current certification as a Certified Nurse Practitioner. Current State license as a Registered Nurse. Current BLS. Current ACLS/ATLS/PALS provider status as required by department.



Qualifications:



*NP - Completion of a Nurse Practitioner training program accredited by the Commission on the Collegiate of Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC)


*PA - Completion of a Physician Assistant training program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)


*NP - Certification by a certification body recognized by the State of Michigan


*PA - Certification by National Commission on Certification of Physician Assistants (NCCPA)



Licensure and Certification


*Current licensure as a Nurse Practitioner or Physician Assistant in the state of Michigan


*National certification as an NP or PA (e.g., ANCC, AANP, NCCPA)


*Advanced Cardiovascular Life Support (ACLS) certification required.


*Basic Life Support (BLS) certification required.


*DEA/CSL, required through credentialing.


*DEA - NPs do not have controlled substance licenses, only PAs. When NPs apply, they will enter MI license number instead of controlled substance license since they won't have a CSL.




Experience


*Must have a minimum of 1-year of experience as an APP in cardiology.


*At least 3-years of cardiology experience in an undergraduate or graduate role.


*Experience in managing heart failure patients is preferred.


*EPIC Experience, preferred.
