Job summary

University of Minnesota Physicians (M Physicians) seeks a Certified Nurse Practitioner or Certified Physician Assistant to join the Heart Care - Heart Failure team at the University of Minnesota Medical Center in Minneapolis.

You will deliver care for adults with heart failure, including transplant and MCS therapies, perform independent clinic follow-ups, and coordinate tests and hospital admissions. We offer competitive compensation and a comprehensive benefits package.

Qualifications

• Graduate of an accredited Physician Assistant or Adult Nurse Practitioner program.
• Licensed or eligible for MN PA or NP license and prescriptive authority.
• Eligible for credentialing and privileges.
• Excellent communication and teamwork in HF/cardiology settings.

Responsibilities

• Provide care for adults with Heart Failure including transplant and MCS therapies.
• Perform independent clinic follow-up.
• Participate in inpatient rounds and coordinate tests and care plans.
• Communicate effectively with hospital staff and patients.
• Utilize EPIC EMR for documentation and orders.
