Job Description
We are seeking a highly organized and emotionally intelligent Remote Medical Assistant – Healthguide Supervisor to lead a team of remote medical assistants – Healthguides conducting patient outreach for highly impactable and transition-of-care patients. This role is critical in addressing social determinants of health (SDOH), resolving access barriers, and ensuring high-touch patient engagement in a fast-paced, mission-driven environment.
What You'll Be Doing
- Supervising, mentoring, and providing ongoing training for a team of remote medical assistants – Healthguides conducting patient outreach and interacting with primary care practices.
- Monitoring team performance, ensuring high-quality interactions and adherence to protocols.
- Fostering a supportive and collaborative remote work culture that prioritizes patient-centered care.
- Providing real-time guidance on handling complex patient needs, including social determinants of health and care coordination challenges.
- Conducting regular team meetings, performance reviews, and professional development sessions.
- Overseeing outreach to highly impactable patients with complex medical and social needs and transition-of-care patients, ensuring timely follow-up and intervention.
- Ensuring staff effectively identify and address patient barriers to care, including transportation, medication access, food insecurity, and other SDOH factors.
- Implementing and monitoring patient engagement strategies that emphasize empathy, active listening, and problem-solving.
- Supporting escalation pathways for complex cases, collaborating with care management teams, social workers, and clinical providers as needed.
- Participating in the development of workflows, scripting, and documentation standards for patient outreach.
- Monitoring call metrics, documentation accuracy, and team productivity.
- Ensuring compliance with HIPAA, patient confidentiality, and organizational policies.
- Understanding data analytics tracking outcomes, identifying trends, and driving continuous improvement.
- Collaborating with cross-functional teams to refine processes and integrate best practices.
- Managing workforce operations, including overtime oversight.
- Ensuring adherence to value-based care principles and organizational quality measures.
- Monitoring and reporting on outreach effectiveness, patient engagement rates, and care gap closure.
- Identifying opportunities to improve patient experience and streamline processes.
Qualifications
- Certified/Registered Medical Assistant (CMA/RMA) or equivalent clinical background.
- 3+ years of experience as a Medical Assistant, including patient outreach or care coordination.
- 1+ years of experience in a supervisory or leadership role, preferably in a remote setting.
- Strong understanding of SDOH and barriers to healthcare access.
- High emotional intelligence (EQ) with excellent communication, coaching, and conflict-resolution skills.
- Ability to thrive in a fast-paced, high-touch environment with shifting priorities.
- Proficiency in electronic health records (EHRs), telehealth platforms, and data tracking tools.
What We'd Love You To Have
- Experience in population health, case management, or value-based care models.
- Familiarity with motivational interviewing techniques and patient-centered engagement strategies.
- Bilingual (Spanish or other languages) a plus.
