Job summary

Inspira Health Network in New Jersey seeks a Master’s level Social Worker to provide psychosocial assessments and discharge planning for high-risk patients across multiple programs (dialysis, mother-baby, oncology). The role collaborates with the interdisciplinary team to educate patients and caregivers and to connect them with community resources for safe discharge.

Qualifications

• Graduate from an accredited School of Social Work with an MSW degree.
• Current licensure as a Social Worker in New Jersey (LSW required).
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) depending on years of social work experience and clinical courses.
• Case Management Certification (ACMA or CCM) preferred.
• Minimum of one year of post-master’s in social work in the healthcare setting preferred.
• Strong writing and effective communication skills; ability to work as part of a multidisciplinary team.

Responsibilities

• Provide psychosocial assessments for patient populations (dialysis, mother-baby, oncology).
• Collaborate with interdisciplinary team in discharge planning for high-risk patients.
• Participate in interdisciplinary rounds; educate patients and caregivers on community resources.
• Connect patients and families with appropriate community resources and agencies to ensure safe discharge.
