Qualifications

• Master’s Degree in social work is required.

• 2 years of home care, hospice, discharge planning or related experience is preferred.

Responsibilities

• Provide counseling or discharge planning to patients and families.
• Assist with care management and behavioral health/social work support.
• Collaborate with care teams to coordinate patient services and resources.
