Job summary

SNF Healthcare in New Jersey is seeking a full-time Clinical Liaison to foster relationships with physicians, discharge planners at hospitals, and community leaders to promote referrals and admissions.

You will conduct pre-admission assessments of residents referred for care, act as a resourceful problem solver, and collaborate with a team to meet targets. The role requires a personable, self-driven professional with LTC liaison experience preferred.

Qualifications

• Preference given to someone with at least 1 year of long term care clinical liaison experience.

Responsibilities

• Fostering sales relationships with physicians and discharge planners at hospitals to ensure referrals/admissions.
• Conducting pre-admission assessments of the clinical status of a resident referral.
• Fostering relationships in the community and local businesses.
