Qualifications

• Experience in healthcare marketing, business development, clinical liaison, admissions, or related field preferred.
• Experience in skilled nursing, rehabilitation, post-acute care, home health, hospice, or hospital settings favored.
• Understanding of discharge planning and post-acute referral process.

Responsibilities

• Develop and maintain relationships with hospitals, physicians, discharge planners, social workers, and other referral sources.
• Conduct regular hospital, physician-office, community, and referral-source visits.
• Generate and cultivate new referral opportunities and manage existing relationships.
• Follow up on referrals and assist with admissions processes.
• Educate partners about the facility's capabilities and programs.
