Job summary

Jaybird Senior Living is seeking a Healthcare Coordinator, LPN/RN, to partner with the Community Director in delivering exceptional care to residents. The role oversees care programs, leads Resident Assistants, coordinates healthcare staff, and ensures HIPAA and state compliance.

Responsibilities include creating individualized care plans, maintaining EHRs, and supervising staff. This on-site position offers a $10,000 sign-on bonus and opportunities for growth within a fast-growing organization.

Qualifications

• Licensed Practical Nurse (LPN) in good standing with the state.
• Current CPR certification.
• Minimum 2 years’ experience in senior living or long-term care, preferably with dementia care.
• Strong organizational, leadership, and interpersonal skills.
• Ability to work flexible schedules, including weekends and occasional overnight travel.

• Notify healthcare providers and families of resident health changes.
• Recruit, train, supervise, and evaluate healthcare staff.

• Ensure proper use of personal protective equipment and safe handling of hazardous materials.
• Provide 24/7 on-site coverage for emergencies and urgent resident needs.
• Document incidents and follow up as required.
• Maintain budget compliance for healthcare operations.

The Healthcare Coordinator partners with the Community Director to deliver exceptional care to residents. This role oversees care programs, leads and trains Resident Assistants, coordinates healthcare staff, and ensures compliance with state regulations, company standards, and HIPAA guidelines.
