Job summary

COPAL is hiring a Health and Wellness Coordinator & Case Manager to plan, coordinate, and execute health and wellness initiatives. They will manage program activities, establish partnerships with community organizations, support community outreach and advocacy efforts, and actively engage community members to support them in accessing resources and navigating health systems.

This is an on-site, full-time role requiring travel across the Twin Cities metro area and occasional evenings/weekends to

Qualifications

• Bachelor’s degree in public health, social work, psychology, counseling, or related field or 1–3 years equivalent experience.
• Bilingual in Spanish and English with ability to communicate effectively with community members.
• Experience in social services, outreach, case management, or nonprofit work preferred.

Responsibilities

• Plan and coordinate program activities with H&W team.
• Coordinate health fairs, vaccination events, workshops, mental health programs and campaigns.
• Manage timelines, resources, and logistics within budget.
• Support outreach strategies to engage target populations.
• Provide intake, assessment and connection to partner resources.
• Deliver trauma-informed, culturally responsive support.

Education
