About the role:

This position is an integral part of Premise’s Growth and Development team, supporting Operations and Account Development on client strategy and delivery for clients with Premise’s population health products (primarily Population Health Insights and Care Management). The Health Strategy Associate (HSA) leverages data to identify key population insights and recommends strategies to drive utilization of Advanced Primary Care and reduce total cost of care.

The role is a perfect fit for someone who excels at building relationships, is highly analytical, and is excited about being part of a close-knit team that is redefining the way primary care is delivered. Successful candidates will have a passion for healthcare, data, and client success.

Essential Functions:

  1. Identify key population insights and outline strategies to drive utilization of Advanced Primary Care products and reduce total cost of care for a client book of business.
  2. Leverage reporting, available data and analytical tools (claims platform, Epic, etc.), and other inputs to drive strategy.
  3. Plan and support the execution of claims-driven outreach and member marketing campaigns to impact clients’ high-cost, high-risk members.
  4. Support the execution of Population Health Insights and Care Management.
  5. Develop a thorough understanding of each client, including all population nuances, and provide recommendations to tailor strategy and optimize product delivery accordingly.
  6. Collaborate with operations and account development to evaluate client adoption trends and identify opportunities for deeper engagement, higher utilization, and potential product upsells to drive greater value for the client and at Premise Health.
  7. Establish and deepen relationships with assigned internal stakeholders at each client by understanding their business needs, priorities, and all aspects of site functions.
  8. Facilitate creation, review, and delivery of client-facing reporting on a quarterly and annual basis, including data-driven ROI/Annual Report.
  9. Support implementation efforts in partnership with operations and product teams.
  10. Partner with data analysts to synthesize findings from analytics and reporting, and derive strategic insights to inform detailed action plans.
  11. Develop a thorough understanding of Premise’s operational model, Growth & Development team goals, total cost of care, and value-based care concepts.
  12. Escalate issues appropriately and collaborate with leadership and cross-functional teams to provide feedback.
  13. May require other duties as assigned.

Job Requirements:

  1. BA/BS required (MBA/Master’s Degree considered a plus).
  2. 1-3 years of experience in strategic and consultative relationships and/or analytical roles.
  3. Experience developing and presenting analyses and recommendations to stakeholders.
  4. Experience establishing and maintaining relationships with individuals at all levels of an organization.
  5. Experience in a healthcare company and/or management / strategy consulting firm strongly preferred.

Preferred Experience:

  1. Intrapreneurial spirit (entrepreneurial spirit with ability to thrive in a large organization).
  2. Excellent problem-solving skills, demonstrated analytical abilities, and the ability to work both independently and within teams.
  3. Superior verbal and written communication skills, with excellent presentation skills, confident in all settings with individuals at all levels of the organization.
  4. Ability to choose the right communication channel depending on the problem to be solved.
  5. Comfort with learning new analytical tools.
  6. Ability to create structure in ambiguous situations and design effective processes.
  7. Advanced level skill in Microsoft Office Suite.
  8. Familiarity with data visualization tools (e.g., Power BI, Qlik) considered a plus.

While a number of factors influence salary, our estimated California, Colorado, Washington, and New York compensation is $60,000 - $77,000.
