Health Services Coordinator/Charge Nurse (LPN/RN) - $3000 Sign on Bonus!!

Position Overview

We are seeking a dedicated Charge Nurse/Health Services Coordinator (HSC) to support the well-being of our assisted living residents. LPN/RN license required. The available shift is Days, including every other weekend and on-call rotation. $3000 Sign-on Bonus!!

As an HSC, you will ensure residents receive high-quality care that meets their physical, emotional, spiritual, and medical needs. You will lead, train, and support caregiving staff authorized to provide care and administer medications, in compliance with state regulations. Working closely with the Health Services Director (HSD), you will help maintain adherence to regulations while upholding care standards. You will also foster a positive environment by delivering exceptional customer service to residents, families, staff, visitors, and vendors.

Essential Functions
  • Assess potential and current residents to ensure appropriate placement and develop care plans, meeting with residents and/or families to discuss the Plan of Care.
  • Support the HSD in recruiting, screening, and selecting compassionate care staff.
  • Ensure staffing levels are adequate to provide high standards of care.
  • Lead care staff by setting performance expectations, providing training, and participating in performance appraisals.
  • Maintain current knowledge of policies and procedures, ensuring compliance with regulations.
  • Oversee medication administration, documentation, and safety policies.
  • Conduct rounds to ensure resident safety and meet their needs.
  • Monitor vital signs, document appropriately, and respond to medical occurrences.
  • Assist with personal care activities as needed and maintain confidentiality and residents’ rights.
  • Communicate effectively with residents, families, and staff, maintaining a positive environment.
  • Participate in emergency procedures and maintain a professional appearance and demeanor.
Managerial Scope

Provide oversight of direct care staff, supporting the supervision of the Health Services department as requested, including staffing, training, and compliance activities.

Qualifications
  • Current RN, LPN, or LVN license per state guidelines.
  • Proven leadership experience in a senior living care environment.
Physical and Work Environment

Work involves physical activities including lifting up to 50-100 pounds, assisting residents with mobility, and performing strenuous tasks. The environment includes residents with memory issues or requiring assistance, with safety and risk management as priorities.
