- Make patient outreach calls and follow up with provider offices to support care coordination
- Use provider portals to research and identify providers within members' service areas
- Assist communication between patients and providers
- Complete administrative tasks related to outreach and health plan support
- Research patient concerns and provide resolutions in collaboration with team members
- Collect and document client data in the medical record
- Coordinate multiple appointments using scheduling systems
- Provide care-gap lists to the multidisciplinary team and coordinate closure of gaps/issues
- Follow up with members, assess needs and obstacles, and address health literacy, cultural issues, and social-class barriers
- Meet and accompany patients and families to initial provider appointments around acute and post-acute transitions
- Establish collaborative relationships with physicians, payers, and healthcare team members
- Collect and communicate timely information to payers to fulfill utilization and regulatory requirements
- Educate internal healthcare team members on care management and managed care concepts
- Work independently while providing an excellent patient-care and customer-service experience
Requirements
- High school diploma/GED or equivalent working knowledge
- Knowledge of medical terminology, normally obtained through Medical Assistant, Patient Care Technician, or Certified Nursing Assistant training
- 2 years of health care experience
- Excellent time management and problem-solving skills
- Ability to meet constantly shifting deadlines and timelines
- Excellent customer service and recovery skills
- Completion of post-offer Occupational Health physical assessment, drug screen, and background check
- Must reside in Arizona
- Bachelor's degree in Social Work preferred
- Additional related education and/or experience preferred
