Job summary

Fairfield Community Health Center in Lancaster, Ohio is seeking a Health Navigator to support PCMH and HRSA program requirements, coordinating care, improving access, and reducing barriers for patients. You will work with the care team to ensure culturally responsive, patient-centered services and timely follow-up.

The role emphasizes population health management, team-based care, and ongoing quality improvement within a compassionate community health setting.

Qualifications

• High school diploma or GED required.
• Graduate of a Medical Assistant program; LPN required.
• CCHW credential preferred, or willingness to obtain certification within established timeframe.
• Minimum of one year of experience in ambulatory primary care or related healthcare setting; FQHC preferred.

Responsibilities

• Facilitate timely access to primary and preventive care services.
• Coordinate referrals, transportation, hospital/ED discharges, and follow-up appointments.
• Provide navigation and scheduling support to reduce barriers to care and assist in outreach.
• Serve as a liaison between patients, families, providers, care teams, and community partners.
• Participate in daily huddles and care team meetings; support population health workflows.
