• Walk the Aberdeen, SD floor at handoff and assess patients before sign-out.
• Educate newly diagnosed patients on self-management prior to leaving Aberdeen, SD.
• Cover weekend rotations with Dell's mid-level clinicians as needed.
• Coordinate discharge planning with case management, social work, and family.
