Fort Defiance Indian Hospital Board, Inc. seeks a Health Information Technician (Rotating) to perform comprehensive medical records reviews and support RPMS, EHR, and imaging systems on-site in Fort Defiance, AZ.

The role requires 1 year of HIM experience, a high school diploma or GED, and adherence to Navajo/Indian Preference policies. Applicant must have a valid unrestricted driver’s license and be prepared for annual training and quality assurance tasks.

Qualifications

• One (1) year direct work experience with Healthcare Information Management functions.

Responsibilities

• Performs a comprehensive review and verification of medical records to ensure all patients’ discharge records meets accreditation requirements.
• Provides qualitative analysis and evaluates medical records for internal consistency and completeness.
• Reviews and analyzes records to ensure correlation of laboratory tests, procedures, and treatments with diagnosis.
• Determines if final diagnosis and procedures stated by the physician are valid and complete.
• Maintains statistics concerning ADT to ensure accurate patient census.
• Enters data into RPMS/ADT and maintains outpatient surgery information.
• Monitors, logs, and distributes transcribed reports to physicians.
• Coordinates with IT staff to update RPMS software.
• Scans patient information into EHR with accuracy and timeliness.
• Imports patient information through Health Information Exchange into EHR and imaging systems.
• Creates Personal Health Record accounts and checks Direct Messaging daily.
• Processes Release of Information requests within 30 days and tracks disclosures.
• Ensures inquiries are answered promptly and accurately.
• Trains new staff on medical record functions.
• Participates in quality improvement monitoring.
• Converts paper records to electronic format using EHR, RPMS, and Vista Imaging.
• Registers patients using RPMS and retrieves medical records.
• Completes birth/death certificate registrations as required.
• Maintains logs and logs master patient index.
