Job summary

Medina Healthcare System is seeking a detail-oriented Medical Records Clerk to compile, verify, and file patient records on-site in Hondo, TX. This role involves assembling diverse records for EMR entry and ensuring HIPAA compliance and timely response to requests.

The position requires experience with HIPAA, PHI release, and EMR systems; RHIT/RHIA credentials are preferred. Monday–Friday, days, with on-site coverage as needed.

Qualifications

• Min 3 years' HIPAA and release of PHI experience, incl. subpoenas and other legal requests.
• Min 2 years in a hospital/clinic setting using EMR system.
• RHIT/RHIA credentials preferred.

Responsibilities

• Prepares and maintains medical records for all types of patients.
• Reviews and assembles records into standard order to scan into EMR.
• Resolves record deficiencies in EMR and responds to requests per policy.
• Monitors delinquencies.
• Posts results of testing and procedures to EMR.
• Interacts with Medical Staff to complete records.
• Answers telephone calls promptly and courteously.
• Knowledge of HIPAA, Release of PHI to law firms, subpoenas, and insurers.
• Prepares birth certificates in TER-TxEver Birth Registry, obtains signatures and releases to state.
• Provides adequate office coverage to respond to department needs.

Job Details: Job Location: Medical Records Building - Hondo, TX 78861, Position Type: Full Time, Travel Percentage: None, Job Shift: Days, This position is full-time, Monday- Friday, 8a-5 pm on-site position.

POSITION PURPOSE:

Compiles, verifies, prepares, and files medical records of hospital or other health care facility by performing the following duties:

KEY RESPONSIBILITIES:

• Prepares and maintains medical records of all types of patients.
• Reviews and assembles all types of medical records into standard order to scan into our EMR system.
• Resolves record deficiencies within the EMR system. Responds to all requests for medical records according to established policies and procedures in a courteous and timely manner.
• Monitors delinquencies.
• Posts results of testing and procedures to EMR.
• Interacts with the Medical Staff for completion of medical records.
• Answers telephone calls promptly and courteously.
• Knowledge of HIPAA, Release of Protected Health Information to Law Firms, Subpoenas, and Insurance providers.
• Prepares birth certificates in our TER-TxEver Electronic Birth Registry system, obtains parents signatures and releases to state in accordance with policy and procedures.
• Provides adequate office coverage to respond to the needs of the department.

Qualifications: QUALIFICATIONS & EDUCATION:

• Minimum of 3 years' experience with HIPAA and Release of Protected Health Information to include legal requests such as subpoenas and other legal entities.
• Minimum of 2 years’ experience in a hospital/clinic setting using EMR System preferred.
• RHIT/RHIA Credentials preferred.
