Job summary

Grace Health is seeking a customer service-minded individual to join the Medical Records Department on a full-time basis. The role emphasizes accurate record handling and strong service to patients and external partners, with a benefits package and no night or weekend work.

The position requires a high school diploma or GED and prefers at least one year of medical records or health information experience, plus basic medical terminology knowledge.

Qualifications

Responsibilities

• Scans, indexes, and distributes patients’ health records and documentation via EHR/EDR or paper.
• Analyzes incoming communications to determine routing per procedures.
• Handles requests for release of medical information according to policy and law.
• Responds to requests from outside entities for auditing or legal purposes.
• Coordinates tracking and completion of forms regarding patient health information and obtains authorizations.
• Provides excellent customer service via verbal, written, and electronic communication to patients and internal/external stakeholders.
• Reconciliation and order matching for messages and results received electronically.
• Maintains the integrity of the master patient list and patient records.
• Uses technologies to analyze, report, and improve patient access to information.
• Documents activities in patient charts.

WHAT YOU WILL DO:

1. Scans, indexes, and distributes patients’ health records and documentation via Electronic Health Records (EHR), and Electronic Dental Records (EDR) systems or by paper.
2. Analyzes incoming electronic and written communications to determine appropriate routing of information. in accordance with established policies and procedures.
3. Handles requests for incoming and outgoing release of medical information according to legislative requirements and policy and procedures.
4. Responds to requests from outside entities for the purpose of auditing or legal proceedings.
5. Coordinates tracking, distribution, and completion of forms regarding patient health information. This includes completing sections of forms not requiring provider review or decision making and preparing forms for provider review and approval. Makes patient contact to obtain authorization for requests of information and sends completed forms to the requested destination.
6. Provides excellent customer service via verbal, written and electronic communication to patients, internal customers, vendors, payers, and healthcare organizations in accordance with established policies and procedures.
7. Reconciliation and order matching for messages and results received electronically.
8. Maintain the integrity of the master patient list and patient medical records.
9. Utilize technologies to analyze, report, and improve patient access to information.
10. Documents activities in patient charts.
