Job summary

DHSS in Delaware seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for SNAP, Medicaid, CHIP, GA, TANF, and other needs-based benefits. You will counsel clients, manage cases, and refer to additional services as needed.

You will work with TANF and SNAP vendors, maintain case records, coordinate with agencies, and use automated systems to research and report health and social data.

Qualifications

• Six months' experience in case management including assessing, planning, developing, implementing, monitoring, and evaluating options and services.
• Six months' experience making recommendations as part of a client’s service plan such as clinical treatment or eligibility determination.
• Six months' experience interpreting laws, rules, regulations, standards, policies or procedures.
• Six months' experience using automated information systems to enter, update, modify, delete, retrieve and report data.

Responsibilities

• Interviews clients to determine needs for health and human services.
• Develops cost-effective service plans using in-house sources and referrals.
• Monitors and assesses service delivery for quality and efficiency.
• Maintains case records, reports, and required documentation.
