Job summary

DHSS in Delaware seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for needs-based benefits, including SNAP, Medical Assistance, CHIP, and TANF. You will counsel clients, coordinate services, and refer to additional supports as needed.

Strong case management, documentation, and cross‑agency collaboration are essential. The role emphasizes timely, accurate information sharing with clients and agencies, with a focus on vulnerable populations and service

Qualifications

• Six months' experience in case management including assessment, planning, implementation, monitoring and evaluation.
• Six months' experience in making recommendations as part of a service plan (e.g., eligibility for benefits).
• Six months' experience interpreting laws, rules, policies, standards and procedures.
• Six months' experience using an automated information system to manage data and generate reports.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients, families, or sources to determine needs for health and human services.
• Develops cost-effective service plans utilizing in-house and contracted providers.
• Monitors service delivery and adjusts plans for quality and efficiency.
• Maintains case records, summaries, and required forms.
• Establishes relationships with clients, families and community sources.
• Provides information and referrals to connect clients with needed services.
