Job summary

Harmonia Collaborative Care is seeking a Health Homes Case Manager to support individuals by coordinating medical, behavioral health, and community services. This full-time, community-based role emphasizes compassionate care, structured scheduling, and a manageable caseload.

The ideal candidate will conduct home visits, develop person-centered care plans, and connect members with essential resources while maintaining accurate documentation and ensuring compliance.

Qualifications

• Bachelor’s degree in human services or related field preferred.
• Experience in case management or behavioral health preferred.
• Strong communication, organization, and documentation skills.
• Reliable, compassionate and professional demeanor.
• Valid NY State driver’s license and reliable transportation.
• Availability Monday–Friday, starting at 7:30 a.m. (37.5 hours/week).

What We're Looking For

• Computer proficiency, including Microsoft Word and Excel.
