Job Summary

Under the general supervision of the PTW Associate Vice President, the Health Home Supervisor provides clinical oversight for PTW Care Coordination and Outreach Services for children and adults. The HH Outreach Supervisor is responsible for assigning and monitoring new outreach and enrollments, ensuring clients are connected to appropriate services. Responsibilities include reviewing caseloads bi-weekly, ensuring complete and timely documentation, and maintaining quality standards. Weekly supervision focuses on clinical issues and chart reviews. The supervisor establishes systems to ensure team operations meet or exceed agency and regulatory standards, including documentation and billing compliance.

Essential Job Functions
  1. Provides Clinical Oversight to Team Members
    • Oversees intake and screening activities to ensure appropriate enrollment.
    • Participates in community outreach and fairs to support enrollment.
    • Supervises assessments, reassessments, and ensures accuracy of Medicaid and demographic info.
    • Reviews referrals and new enrollments for eligibility and linkage to services.
    • Supervises development and maintenance of Care Plans and Assessments.
    • Supervises crisis intervention services and reviews crisis management plans.
    • Ensures timely delivery of quality services and client satisfaction.
    • Supervises counseling and exit planning, ensuring proper discharge procedures.
    • Reports serious incidents and completes incident reports per policies.
    • Conducts supervision, case conferences, and staff development activities.
  2. Directs and Supervises Personnel
    • Manages staff performance, provides feedback, and handles disciplinary actions.
    • Monitors employee morale, addresses harassment and discrimination complaints.
    • Oversees employee leave requests and maintains contact during FMLA leave.
    • Oversees administrative record keeping, billing, and equipment maintenance.
  3. Provides Quality Assurance Oversight
    • Ensures compliance with regulatory requirements and monitors service quality.
    • Develops program mission and goals aligned with agency objectives.
    • Prepares for audits and develops corrective action plans.
    • Ensures accurate and timely submission of statistical data.
  4. Other Miscellaneous Duties
    • Performs other related duties as assigned.
Essential Knowledge, Skills, and Abilities
  • Strong promotion of ICL values and goals.
  • Knowledge of community resources supporting treatment plans.
  • Leadership skills for managing multidisciplinary teams.
  • Understanding of comprehensive Care Plans.
  • Proficiency in electronic health records and technology.
  • Knowledge of policies, regulations, and legislation.
  • Supervision principles and practices.
  • Knowledge of evidence-based treatment practices.
  • Resource management skills.
  • Excellent communication skills.
Qualifications and Experience

Licensed by the NYS Education Department as a mental health practitioner (e.g., Social Worker, Marriage and Family Therapist, Creative Arts Therapist, Psychologist) with at least 1 year of supervisory experience in a behavioral health setting.

OR

Master’s degree in a mental health discipline with 3 years of supervising clinicians or case managers providing services to individuals with SMI or SUD.
