Job summary

FreedomCare is seeking a dedicated Case Manager for our Care Management team in upstate NY. This role involves outreach to eligible patients, enrollment in Health Home Care Management, and coordination of comprehensive, patient-centered care plans with the care team.

You will travel to patient homes in Buffalo and surrounding Erie County areas to complete assessments and coordinate services. Ideal candidates have 1+ year of Health Homes or case management experience, strong knowledge of local

Qualifications

• 1+ year of Health Homes, HARP or case management experience (required).
• Strong working knowledge of local community resources.
• Demonstrated ability to work with data reporting, documentation, and outcomes.
• Ability to travel in the field to accompany patients to appointments and meet patients in person when needed.
• Excellent communication and assessment skills; ability to relate to patients, families, and community care providers, and handle crisis situations.
• Ability to manage high volume caseloads.
• Experience navigating data management systems such as Salesforce.

Responsibilities

• Screen for Health Home eligibility and conduct initial and reassessments of patient needs.
• Collaborate with medical providers and patients to develop, implement, and coordinate Health Homes compliant care plans.
• Provide direct service to a caseload and facilitate referrals, address access issues, and connect patients to local resources.
• Coordinate patient services with pharmacies, insurance, hospital discharge planning, family caregivers, and other providers.
• Provide crisis intervention when needed.
• Conduct home visits and maintain patient documentation per Health Homes standards.
• Maintain patient confidentiality at all times.
