Job summary

FreedomCare is seeking a Case Manager for our Care Management team in upstate NY. The role requires frequent travel to patients’ homes in Buffalo, NY and surrounding Erie County. You will assess patients, coordinate plans, and collaborate with care teams to ensure high-quality care.

The ideal candidate has 1+ year in Health Homes or case management, strong communication skills, and experience with data reporting. A Bachelor's degree is preferred and bilingual Spanish is a plus.

Qualifications

• 1+ year of Health Homes, HARP or case management experience (required).
• Strong knowledge of local community resources.
• Experience with data reporting, documentation, and outcomes.
• Ability to travel to patients' homes and meet them in person when needed.
• Bachelor's degree preferred; associate degree acceptable.

Responsibilities

• Screen for Health Home eligibility and conduct initial assessments and reassessments.
• Develop and coordinate Health Homes compliant care plans with providers.
• Provide direct service to a caseload and facilitate referrals and access to resources.
• Coordinate patient services with pharmacies, insurers, hospitals, and caregivers.
• Provide crisis intervention when needed.
• Conduct home visits and document services per Health Homes standards.
• Maintain patient confidentiality at all times.
