Medium is seeking a Medical Care Manager to coordinate high-quality healthcare services for individuals in the Health Home program. This role requires strong clinical coordination skills and the ability to advocate for individual clients' needs.

The candidate should have a Bachelor's or Associate Degree in a health-related field and possess a Maryland RN license. Prior experience in Human Services or Healthcare is preferred. This position offers a salary of $30-$32 per hour.

Formación

• Minimum of one year of experience in Human Services, Behavioral Health, or Healthcare.
• Ability to work independently and prioritize tasks.

Responsabilidades

• Support coordination and delivery of healthcare services.
• Manage laboratory testing and toxicology screenings.
• Advocate for individuals regarding their healthcare needs.

Conocimientos

Care coordination and advocacy skills

Knowledge of medical terminology

Proficiency in Microsoft Office

Strong organizational skills

Educación

Bachelor's Degree or Associate Degree in Health Administration or related field

Current Registered Nurse (RN) license in Maryland
