Qualifications

• High School diploma or GED required.
• Bachelor’s degree in Health or Human Services preferred.
• Minimum 2 years of related work experience.
• Full-time position with hourly pay of $23.00–$26.37.

Responsibilities

• Coordinate care plans with an interdisciplinary team.
• Direct field activity with the Patient Navigator to optimize care flow.
• Develop written individualized care plans with providers/patients.
• Review intake assessments to support care plans and goals.
• Identify barriers to care and help patients overcome them.
• Ensure medication reconciliation is current and assess adherence.
• Participate in discharge planning and care transitions.
• Follow up with patients within one business day after discharge.
• Arrange preventive screenings and educate on self-management.
