Job summary

Jewish Family Services of Western New York in Buffalo, NY is seeking a Care Coordinator-HH+ to manage Health Home Plus caseloads, provide enhanced care coordination, and help clients access HCBS and CORE services.

The role requires a Bachelor's degree in a health/social field and two years' experience with SMI/behavioral health clients; bilingual skills are a plus. The position offers a competitive salary and benefits, with a 35-hour work week and strong team support.

Qualifications

• Bachelor's degree in a related field and relevant health services experience.
• Two years of direct service experience with mental health, SMI, or related populations.
• Proficiency with health information systems and MS Office tools.

Responsibilities

• Maintain a caseload of HH+ and HARP clients with care coordination.
• Determine HH+ eligibility and conduct home visits in the community.
• Develop and maintain a Plan of Care addressing client needs.
• Complete and review crisis plans and annual case reviews.
• Document progress notes within 48 hours of encounters.
• Collaborate with care teams to link clients to HCBS and CORE.
