Job summary

Jobtailor seeks a dedicated Care Coordinator to provide Health Home services and coordinate care for patients across home, community, clinics, and inpatient settings. You will collaborate with care teams to develop Health Action Plans and monitor progress toward goals.

Candidates should have a background in social services or health sciences, strong organizational and communication skills, and be comfortable with chronic condition terminology. CPR certification is required during probation.

Qualifications

• BSW or BA/BS in Human Services, Health Sciences or related field with experience in social service case management or care coordination.
• Strong organizational and communication skills; professionalism and appropriate boundaries in all interactions.
• Must understand medical terminology pertaining to chronic conditions.
• Bilingual English/Spanish preferred.
• CPR certification within probationary period and maintained throughout employment.

Responsibilities

• Provides Health Home services and similar supports for patients.
• Meets with patients in the location of their choice; homes, community, in-patient settings or clinics.
• Works with the patient to create a Health Action Plan including long and short term goals.
• Follows up with the patient regularly to evaluate progress towards goals.
• Participates in case review and multidisciplinary meetings with the client’s care team.
