Description

Whitney Young Health (WYH), in partnership with Capital Region Health Connections, is committed to providing comprehensive community-based case management services to adults with chronic medical conditions and co-occurring mental health and substance abuse disorders, through Health Home Care Coordination across Albany and Rensselaer counties.

If you possess a desire to provide quality community-based linkages, referrals, and service planning for individuals with chronic medical conditions and co-occurring mental health and substance abuse disorders, consider joining the WYH team as a Health Home Care Coordinator.

Job Summary

Using a patient-centered approach, Health Home Care Coordinators assist individuals enrolled in the program to manage their conditions and achieve optimal health outcomes by developing individualized care plans. These plans are designed to link individuals to necessary services such as housing, healthcare, mental health and substance use providers, social services (food, entitlement benefits, transportation), community programs, and more.

The ideal candidate possesses excellent written and verbal communication skills, knowledge of community resources, and strong time management and organizational skills. The candidate should demonstrate computer proficiency, good judgment, establish healthy boundaries, work independently, and adhere to strict confidentiality guidelines.

Requirements

Minimum Qualifications

  • Associate’s degree in human services or a related field, with previous experience working with people with behavioral health disorders, HIV/AIDS, children, & families, ID/DD, or substance use disorders.
  • Valid NYS Driver License in good standing (verified annually), and proof of auto insurance in compliance with NYS mandatory limits and coverage.

Preferred Qualifications

  • Bachelor’s degree in a health or human services related field such as psychology, sociology, social work, or nursing, with at least one year of related Health Home Care Coordination work experience in a medical setting.
  • Bilingual (English/Spanish).

Salary range: $21.00 - $24.00 hourly
