Job summary
The Illinois Department of Public Health is seeking a Health Facilities Surveillance Nurse to work in the Division of Intermediate Care Facilities/Individuals with Intellectual Disabilities. You will conduct surveys of facilities for licensure, federal certification, and complaints, and prepare written survey documentation.
Travel to sites is required to determine compliance with state and federal requirements.
Qualifications
- Requires RN license in the State of Illinois.
- Graduation from an approved nursing program with options of associate/diploma, bachelor’s or master’s in nursing.
- Preferred experience in ICF/IID settings and in performing survey-related activities.
Responsibilities
- Conduct surveys of facilities for licensure and federal certification.
- Prepare written survey documentation and findings.
- Travel to facilities, interview residents and staff, and review medical records.
- Ensure compliance with state and federal requirements and agency directives.
