Illinois Department of Public Health is seeking a Health Facilities Surveillance Nurse in Peoria, IL. The role conducts license and certification surveys, reviews records, and ensures facility compliance with state and federal regulations.

Hybrid in-office schedule optional; travel required for site visits. Preferred candidates hold an RN license in Illinois with 3+ years of nursing experience, preferably in long-term care, and strong written/oral reporting skills.

Qualifications

• RN licensure in Illinois required.
• Bachelor’s or higher in nursing preferred.
• Minimum 3 years professional nursing experience; long-term care preferred.

Responsabilités

• Survey long-term care facilities for licensure, certification and compliance.
• Prepare written survey findings and documentation.
• Travel to facilities and interact with residents, staff and administrators.
• Maintain and update survey records and reports.
