Responsibilities

GENERAL DESCRIPTION

The Health Educator is responsible for designing and implementing educational interventions aimed at preventive care, healthy lifestyles promotion and condition management aligned with the goals and objectives of the company’s Wellness Program.

ESSENTIAL FUNCTIONS

• Design and develop individual and group educational interventions based on population needs and program objectives, utilizing evidence-based practices, credible reference sources, and current clinical guidelines for preventive care and chronic disease management.
• Develop and implement metrics and monitoring processes at the individual and group levels to support the design, evaluation, and continuous improvement of educational intervention plans.
• Design, evaluate, and update educational materials in alignment with established objectives, program themes, and target population characteristics.
• Ensure compliance with internal policies and procedures for the development, review, and approval of educational materials.
• Deliver individual and group educational interventions in person across multiple locations, telephonically, or virtually, in accordance with the Wellness Program plan.
• Document all individual interventions accurately and timely within the electronic clinical management system (e.g., Trucare), in compliance with organizational and regulatory requirements.
• Prepare and submit required metric reports in alignment with assigned projects, initiatives, and program goals.
• Identify member needs during interventions and facilitate appropriate referrals to clinical programs and internal or external resources, following established protocols.
• Participate in internal and external meetings as a representative of the Health Promotion and Wellness unit to identify opportunities for educational initiatives and collaborative partnerships that support health promotion and disease prevention.
• Prepare and maintain monthly reports summarizing completed educational interventions and program outcomes.
• Participate in departmental meetings to support the development and implementation of policies, procedures, and strategies related to the Health Promotion and Wellness Program.
• Must comply fully and consistently with all company policies and procedures, with local and federal laws as well as with the regulations applicable to our Industry, to maintain appropriate business and employment practices.
• May carry out other duties and responsibilities as assigned, according to the requirements of education and experience contained in this document.

MINIMUM QUALIFICATIONS

Education and Experience

Bachelor’s degree in Health Education, Public Health, Education in Community Health or related field. At least two (2) years of experience in health promotion, community outreach, or health education roles, preferably within the health insurance industry.

Certifications / Licenses

A valid Health Educator License from the Puerto Rico Department of Health is required. A Driver's License valid in the Commonwealth of Puerto Rico is required. A Certified Health Education Specialist (CHES) credential is considered a plus.

Others

Availability to travel around the Island, conducting occasional field work, and implementing educational initiatives.
