Job Description

THIS POSITION IS REMOTE/WORK FROM HOME IN THE POSTING STATES. THE POSITION STRONGLY PREFERS A REGISTERED DIETITIAN LICENSURE. THE WORK SCHEDULE IS MONDAY - THURSDAY 8AM - 4:30PM OR 8:30AM - 5PM; 2 EVENING SHIFTS 10:30AM - 7PM PER WEEK, AND 1 LAST SHIFT ON FRIDAY/MONTH 9:30AM - 6PM BASED ON CENTRAL TIME ZONE HOURS.

Position Purpose:

Assists members by completing health assessments and implementing effective solutions and care plans to improve their health and wellness. Works with members who are at risk or have been diagnosed with chronic condition(s) to provide support, encouragement, and education.

• Utilize applicable clinical training and knowledge to provide appropriate health support and solutions to members with chronic health conditions
• Evaluate the needs of the member by performing telephonic assessments to determine clinical risk and readiness to change
• Provide telephonic support and coaching for members on ongoing wellness care plans and wellness programs to improve member's health and lifestyle through outreach and education
• Assist members in developing attainable wellness goals through plans, support, encouragement, and resources
• Monitor wellness plans and/or member status and outcomes for changes in care and provide recommendations to care plan based on member needs and issues identified to reduce health risks
• Engage and influence members in decisions relating to achieving and maintaining an optimal healthy lifestyle
• Interact with healthcare providers as appropriate to ensure that wellness plans are aligned with overall medical plans of care
• Collect, document, and maintain member information to ensure compliance with current state and federal accreditation standards
• Provide feedback to leadership on opportunities to improve and enhance care and quality delivery for members in a cost-effective manner
• Perform other duties as assigned
• Comply with all policies and standards

Education/Experience:

Requires an Associate's degree and 2 - 4 years of related experience.

License/Certification:

Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, Master's in Behavioral Health or Health Coach Certification required

Pay Range: $26.50 - $47.59 per hour

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law.
