Job summary

Saint Thomas Community Health Center in New Orleans seeks a Health Coach to partner with providers and nurses to guide patients toward healthier lives. You will build relationships, review care plans, and set achievable goals using Motivational Interviewing in both in-person and phone interactions.

Responsibilities

• Assist to break down barriers for access to healthcare.

Health Coaches are afforded the opportunity to work closely with providers, nurses, medical assistants and case management. The role includes developing relationships with patients, reviewing care plans, and setting goals. The health coach offers supportive guidance to help patients foster healthier lives.
