Job summary

Kids for the Future is seeking a Pediatric Health Care Coordinator (nursing) to lead resident-focused monitoring and data collection in line with physician orders and pediatric care plans.

The role requires nursing licensure in Pennsylvania, CPR certification within 6 months, and ability to work across shifts to ensure seamless care for developmentally delayed children with complex medical needs. Collaboration with the IDT and families is essential.

Qualifications

• Graduate of an accredited school of nursing.
• Must maintain a current Pennsylvania Nursing license (LPN or RN).
• CPR certification within 6 months of employment.
• Exhibits flexibility with regard to scheduled work assignments, overtime, or atypical shifts.
• Is dependable and attends regularly scheduled meetings.
• Must be able to read, write and understand work instructions in English.
• Must be familiar with Microsoft Office.
• Ability to lift/carry >50 lbs and move up to 200 lbs with assistance.
• Standing, walking, bending, twisting and adaptability to changing hours.

Responsibilities

• Monitor residents and collect assessment data per physician orders and changes in condition.
• Communicate assessment findings to care teams, physicians, and families.
• Evaluate resident responses to treatments and interventions.
• Ensure nursing documentation meets regulatory and facility standards.
• Coordinate with IDT and nursing leadership to optimize care for pediatric residents.
• Provide supervision and support to staff nurses in pediatric disability care.

Description

Essential Job Functions

• Initiate and maintain ongoing resident-focused monitoring, including the timely, accurate, and appropriate collection of assessment data in accordance with physician orders, prescribed nursing regimens, changes in resident’s condition, or health status. Analyze assessment findings and other clinical data in relation to each resident’s baseline clinical data. Nursing assessments and interventions are substantiated by research findings (i.e. are evidence-based), are appropriate in relation to each resident’s age, developmental level, health status, and medical condition.
• Demonstrates sound decision-making and prudent, reasonable judgments through the execution of timely, accurate, and precise interpretation of nursing assessment findings, and through the timely, accurate, and precise communication of relevant assessment findings via nurse-to-nurse communications, nurse-to-physician communications, nurse-to-Interdisciplinary Team (IDT) communication, and nurse-to-parent / family/guardian communications.
• Evaluate resident responses (i.e. response patterns, or trends) to medical conditions, illness, or pathological processes as well as clinical outcomes and responses to physician prescribed, nursing executed treatments, medications, feeding regimens, therapies, etc.
• Insure that written documentation on medical records meets all regulatory and professional organization standards (i.e. all nursing documentation is completed using the appropriate medical records, assessment tools, flow sheets, and records specified for the purpose, and in accordance with Facility guidelines, policies and/or procedures that guide their specific use.)
• Demonstrates sound judgment regarding resource utilization--considers factors related to safety, appropriateness, effectiveness, and cost in planning, organizing, coordinating, providing and evaluating care for children who are developmentally delayed and medically complex.
• Exercises informed judgment and uses individual knowledge, skills, competencies, and professional nursing qualifications when conducting first-line nursing management responsibilities, and delegating nursing activities to others.

Other Job Functions

• Participation in Nursing Department and facility quality improvement activities, including the collection and analysis of data, formulation of recommendations aimed at ongoing improvement in care, peer review and other means of evaluation to assure continuous improvement in the quality of nursing care.
• Shares knowledge and skills with colleagues, provides staff nurses with constructive feedback and appraisal of their nursing care, and contributes to a teaching-learning environment that is conductive to the clinical education of all facility staff.
• Assists and supports nursing colleagues during their orientation, when the nursing care intensity is unusually high, or whenever additional help is needed.
• Consistently demonstrates “organized,” “efficient,” and “productive” work output in performing job expectations and assigned work responsibilities.
• Collaborates and consults with the Director of Nursing, IDT members, Administrator, direct care staff, and residents’ family members.
• Has the knowledge, critical thinking abilities, clinical and technical skills, interpersonal skills, and competencies needed to provide supervision, direction and oversight of staff nurses in the provision of pediatric disabilities nursing care, including but not limited to providing direct care staff in the provision of residential services and ADLs.
• Ensure the proper function of all medical adaptive equipment.
• Ensure Nurses maintain 7-day supply of all treatments and Medications.
• Medical on-call rotation.
• Provide floor nurse coverage as needed.
• Other duties assigned.
