Horizon House, Inc. is seeking a Full-Time Health Care Coordinator to join our Developmental Services Division.

The role leads clinical care coordination, conducts assessments, and develops individualized care plans for adults with complex medical needs in a community-based setting. You will collaborate with physicians, therapists, and community providers, deliver health education to participants and families, train direct support professionals, ensure regulatory compliance, and strive to

Qualifications

• BSN from an accredited nursing program.
• Current RN license.
• Experience in community health or behavioral health preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and collaboratively.
• Proficiency with electronic documentation and computer systems.

Responsibilities

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,500 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting.

Currently, we are seeking a Full-TimeHealth Care Coordinator to work in our Developmental Services Division.

What You'll Do
