Posting Summary

Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Mercer South for the Child Health Program within the School of Nursing. The Heath Care Case Manager ( HCCM ) functions to support the Department of Children and Families ( DCF )/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.

Among the key duties of this position are the following:

  • Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver and DCF /CP&P, as needed.
  • Assesses a child’s heath care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers as needed.
  • Schedules an Initial Physical Health Evaluation, in consultation with the caregiver and DCF /CP&P, reviews the preliminary and final reports, and ensures that all recommendations for health care are completed.
  • Develops a child health care plan in collaboration with the child’s medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF /CP&P.
  • Links the child and caregiver with appropriate health resources and community services.
  • Assures that the child is scheduled for and attends timely EPSDT visits and specialty appointments, reviews all reports, and ensures all necessary reports are forwarded to the medical home.
  • A Bachelor’s Degree in Nursing or a BA / BS degree in a health related field required.
  • May substitute years of pediatric nursing experience for degree.
  • A minimum of 1-2 years of experience in pediatric nursing required.
Certifications/Licenses
  • Current certification in American Heart Association Basic Life Support for Healthcare Providers required.
  • Vehicle and valid New Jersey license required for off-site visits.
