Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for the Salem Office for the Child Health Program within the School of Nursing, located in Salem, NJ. The Heath Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.
Among the key duties of this position are the following:
Full Time, 37.50 hours per week, Day shift, Exempt (Grade 25S), Salary Range: $70,041 - $99,858 annually, Benefits available, Union: HPAE 5094, Location: Salem, NJ.
