Posting Summary

Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for the Salem Office for the Child Health Program within the School of Nursing, located in Salem, NJ. The Heath Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.

Among the key duties of this position are the following:

  • Assist with enrollment of a child in a managed care entity and identification of a medical home (primary care provider) upon entry into out-of-home placement, and inform the child, caregiver, and DCF/CP&P as needed.
  • Assess a child's health care status and needs by retrieving a complete health history, obtaining and reviewing all medical records, and interviewing past providers and caregivers; forward all information to the medical home and other health providers as needed.
  • Schedule an Initial Physical Health Evaluation, review reports, and ensure all health care recommendations are completed.
  • Develop a child health care plan in collaboration with the child's medical home, specialists, caregivers, community service providers, and DCF/CP&P.
  • Link the child and caregiver with appropriate health resources and community services.
  • Ensure the child attends timely EPSDT visits and specialty appointments, review reports, and forward necessary reports to the medical home.
Position Details

Full Time, 37.50 hours per week, Day shift, Exempt (Grade 25S), Salary Range: $70,041 - $99,858 annually, Benefits available, Union: HPAE 5094, Location: Salem, NJ.

Qualifications

Minimum Education and Experience:

  • Bachelor's Degree in Nursing or a related health field required.
  • Experience in pediatric nursing (1-2 years) required; substitution possible with relevant experience.

Certifications/Licenses:

  • Current BLS certification from AHA required.
  • Valid New Jersey RN license required.
  • Vehicle and driver's license required for off-site visits.

Preferred Qualifications:

  • Two years in nursing case management, community health nursing, or care of vulnerable families preferred.
