ORGANIZATION SUMMARY

Today, organizational programming focuses on humanitarian and disaster response, community strengthening and resilience, and refugee & immigrant services and advocacy.

This position is reliant upon funding and may be subject to modification or termination based on resource availability.

POSITION SUMMARY:

The Health and Wellness Coordinator will provide Preferred Communities intensive case management services to eligible refugees and immigrants in the Triad area of NC. The primary responsibilities of this position will include assisting clients to complete personalized goal plans to remove barriers to self-sufficiency and cultural adjustment, providing support on managing appointments, navigating health and wellness goals, progressing towards naturalization, and connecting clients to appropriate community resources.

This is a limited-term position funded through a grant agreement until September 30, 2025, and contingent upon funding extension.


ROLE & RESPONSIBILITIES:
Client Services:
  • Provide comprehensive case management for enrolled cases to include the following responsibilities:
  • Conduct pre and continued screening with each client referred to the Preferred Communities (PC) program to determine eligibility and conduct follow-up assessments at the 90th, 180th, 270th, and 360th day to monitor client progress
  • Orient and enroll eligible clients into the PC program of ICM and IAR for the Health and Wellness department
  • Guide the goal-setting process with each enrolled client to identify areas of vulnerability and detail action steps necessary to promote self-sufficiency
  • Perform regular home visits and welfare evaluations throughout the program period
  • Assist enrolled clients with accessing necessary services as needed through assisting with completing applications and making referrals
  • Develop and strengthen collaboration with local organizations, social service agencies, and healthcare providers
  • Ensure provision of all necessary services as outlined by the program guidelines, to include accompanying clients to appointments (medical, legal, DSS, etc)
  • Assist clients through advocacy and mentorship in developing the life skills necessary for long-term self-sufficiency
  • Coordinate cross-departmental internal referrals and services at World Relief
  • Coordinate with and guide volunteers and interns through providing direct services to clients as opportunities arise
  • Maintain accurate documentation of client files, case notes, reporting records, and submit monthly program reports
  • Provide mediation services between clients and outside entities (i.e., healthcare providers, volunteers, etc.), and additional adjustment counseling as needed
  • Ensure professional and timely provision of all necessary services as outlined by the PC Guidelines
Administrative and Program Responsibilities:
  • Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
  • Submit any requested data to the program manager through monthly, semi-annual, and annual reports
  • Maintain accurate and organized files and records of services provided
  • Ensure continuity of services and reporting during staff transitions and in the absence of a Program Manager
  • Adhere to all World Relief policies, including but not limited to, incident reporting procedures, code of conduct, security safety and health
  • Travel to and between High Point and Winston Salem office locations is a requirement for this position
  • Perform other tasks and projects as assigned
JOB REQUIREMENTS:PREFERRED QUALIFICATIONS:
  • Degree in Social, Public Health, or similar field.
  • Cross-cultural experience.
  • Bilingual in any of the following languages: Arabic, Swahili, Spanish, French, Kareni, Burmese, Pashto, Dari.

$17 - $19 an hour
