Job description

Health and Wellness Coordinator

Our Assisted Living Health and Wellness Coordinator will support and take direction from our Health and Wellness Director in all health care services throughout the residence. Participate in service planning, as well as renewal and maintenance of these plans as needed. Cooperate with the Health and Wellness Director and the Executive Director to supervise, oversee, and coordinate the work of personal service assistants to meet residents' needs. Provide clinical services to residents and assist in training personal service assistants.

Essential Duties
  1. Support a philosophy that promotes dignity, privacy, independence, choice, and individuality for residents.
  2. Coordinate, in cooperation with the Executive Director, ancillary health care and health education services for residents.
  3. Ensure all required documentation is completed accurately and thoroughly, including service and medication delivery, maintaining records according to regulatory guidelines.
  4. Act as a resident advocate in medical and social situations involving residents, their designated agents, physicians, or other agencies under the direction of the current HWD.
  5. Promote infection control procedures and track trends for Quality Management Review.
  6. Verify physician orders, move-in/move-out information, and initiate appropriate follow-up actions.
  7. Identify resident problems, concerns, or issues and provide appropriate follow-up to ensure resolution.
  8. Be immediately available and awake during nighttime hours as the HW Coordinator.
Education/Experience
  1. Hold a current license as an LPN/LVN or RN, including annual continuing education requirements.
  2. Possess the necessary licensure and examinations per state laws. Prefer prior experience in Assisted Living or Skilled Nursing Units and at least 1 year of experience as an LPN/LVN or RN.

  2. Be a supportive team member, contributing to and exemplifying teamwork.
  3. Ability to make independent decisions when necessary.
  4. Possess patience, tact, a cheerful disposition, enthusiasm, and willingness to handle difficult residents.
  5. Be open to adopting new methods and principles into nursing practices.
