Accompany Health is seeking a Health Advocate to support patients with complex health and social needs in their homes and via virtual visits. You will help build trust, coordinate care plans, and support clinicians across an interdisciplinary team.
Responsibilities include conducting in-home assessments, collecting vitals, facilitating video visits, assisting with social resources, documenting care plans, and coaching patients toward healthier, more connected lives.

At Accompany Health, we’re reimagining primary care for people with complex health and social needs. As a Health Advocate, you serve as the hands, eyes, and ears of our care team-building trust with patients, supporting virtual clinicians, and helping patients advance their care plans.
You’ll visit patients in their homes to facilitate video visits, take vitals, complete screenings, deliver test kits, and follow up on care plan items. You’ll collaborate with an interdisciplinary team of physicians, nurses, behavioral health clinicians, and community health workers to help patients live healthier, more connected lives.
Salary ranges are based on role, level, and location, and reflect the minimum to maximum target for new hire pay in each position. Depending on the role, you may also be eligible for bonus, equity, and benefits. Where you fall within the range depends on factors such as your location, skills, experience, and relevant education or training.
