Job summary

GLEAMNS Human Resources Commission, Incorporated invites applications for a Head Start Case Worker to provide social services and support to families to enhance child development and well-being. The role involves interviewing clients, developing family service plans, and connecting families with healthcare, housing, and other community resources.

The position requires a BS in Social Services or related field, at least two years in social services, and strong computer skills.

Qualifications

• Two years of work experience in a social services environment.
• Must be computer savvy, especially Microsoft products.
• Must be able to work well on a team.

Responsibilities

• Interview clients individually, in families, or in groups, to assess needs and determine required services.
• Develop a coordinated family service plan, including healthcare, for each family.
• Initiate contacts with community support services addressing comprehensive needs.
• Maintain case history records and prepare monthly reports (attendance, screenings, immunizations).
• Refer clients to community resources and provide concrete guidance on where to go and how to apply.
• Serve as liaison between students, homes, schools, and service providers to assist children with disabilities, poverty, or other issues.
• Counsel students and arrange needed services based on assessed problems.
• Organize family meetings, provide training, and keep notes/minutes.
• Arrange follow-up on home visits and monitor family progress.
• Evaluate characteristics to determine needs or eligibility.
• Collaborate with professionals to assess client needs or plan treatments.
• Prepare client file documentation and provide periodic reports to the Family & Community Partnerships Manager.
• Perform other social services as required.
