Job summary

Conduit Health is transforming post-acute care by delivering essential medical equipment and care services to homes through an AI-powered platform. We seek a hands-on manager to lead the Patient Outreach and Intake team, owning strategy, execution, and performance to drive qualified patient experiences and growth across inbound and outbound channels.

You will manage full-time staff and oversee BPO partners, building processes, staffing, and performance infrastructure to scale.

Qualifications

• Experience managing teams in patient operations or other high-volume, performance-driven environments.
• Hands-on management style with coaching, data review, and issue-solving.
• Data-driven track record of improving conversion, productivity, and operational performance.
• Experience establishing SOPs, quality standards, and clear accountability across a team.

Responsibilities

• Own day-to-day Patient Outreach & Intake Operations across inbound/outbound activities and enrollment.
• Lead and scale the team, including hiring, training, coaching, and performance management.
• Build KPI reporting, optimize lead flow, and drive improvements in conversion and throughput.
• Partner with Product to test AI/tools and translate frontline pain points into requirements.
• Own conversion funnel performance and reduce drop-offs through data-driven strategies.
