Under direction of the Intensivist lead for HDU and the HDU Manager the post Holder will have the responsibility for maintaining the HDU clinical databases required for ICNARC (Intensive Care National Audit and Research Centre), CCMDS (Critical Care Minimum data set) and assist in providing reports and feedback to the Clinical leads. As well as other Ward Clerk activities.

Ensure patients are registered onto the Patient Administration System (PAS, LORENZO) and admitted onto ICNARC and CCMDS.
Manually collect and record patients admission, physiology and outcome details from the HDU charts and patients hospital notes for ICNARC & CCMDS data entry.
Liaise with Medical staff to ensure accurate coding of inpatients episodes for ICNARC and CCMDS data entry, Collect relevant information from unit patient prescribing information communication system (PICS)
Liaise with medical records requesting notes collecting discharge information.
Communicate with other hospitals and audit clerks to complete patient outcomes, either by phone calls or using the E mail system
To provide administrative support to the Senior Nurse and assist with ad hoc information requirements.
Prompt completion of the ICNARC Data Validation Report
Provide monthly reports if requested by clinical leads or MDT in support of research projects or Data review.
To analyse data collection and escalate any areas of concern to the Clinical leads.
Show an understanding of medical terminology, anatomical and Physiological terms. Have a basic Knowledge of orthopedic procedures.
To ensure the accurate and timely update of the PAS/Lorenzo system and unit admission book, to include details of admissions, transfers, discharges and any changes to patient details.
To be familiar with the Trusts policies e.g. Infection control, Uniform, and no smoking.
Take responsibility for the collection and disposal of confidential waste.
Undertake general admin support duties for the unit: photocopying, distributing post, laminating etc.
Be conversant with and adhere to the Data Protection Act and protect the confidentiality of patients, relatives and staff within the guidelines (Ref Caldicott)
Be mindful of potentially confrontational situations and where possible attempt to defuse the situation or seek help and advice from senior staff
To undertake any other duties as may be required from time to time which are commensurate with the nature and grading of the post
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£24,625 to £25,674 a yearper annum, pro rata if part time
