Job Summary:

The HCBS Unlicensed Behavioral Health Provider plays a key role in FLACRA’s mission to provide client-centered services as identified and approved by HARP Care Management and Managed Care Organizations (MCO). This position is responsible for delivering specialized Home and Community-Based Services (HCBS) to eligible clients with SUD and/or SMI. Services will be tailored to meet individual needs and adhere to guidelines outlined in the New York State HARP Adult Behavioral Health Home and Community-Based Services Provider Manual.

Required Qualifications:

  • Unlicensed staff must be at least 18 years of age and possess a high school diploma or equivalent, with 1–3 years of relevant experience working with individuals with substance use disorders (SUD) and/or serious mental illness (SMI), or a bachelor’s degree in a related field.
  • Certified Peer Specialist/Certified Recovery Peer Advocate or equivalent qualifications through education and/or work experience, with one year of education substituting for one year of experience.
  • Valid NYS Driver’s License required.

Responsibilities:

Client Support and Service Delivery:

  • Serve as an active member of the client’s treatment team, participating in team meetings and treatment planning alongside care managers and HARP coordinators.
  • Manage individual caseloads, including referrals, intake, prior authorization with insurance providers, and service thresholds.
  • Develop, update, and review individual service and care plans, ensuring alignment with approved HCBS services.
  • Provide direct services as outlined in the BH HCBS Provider Manual, tailored to each client’s needs.
  • Serve as a role model by teaching effective coping strategies, self-help techniques, and recovery processes based on assigned service lines.

Collaboration and Advocacy:

  • Coordinate care and resources for clients, ensuring seamless integration with case management services.
  • Advocate on behalf of clients to help them navigate community services and achieve recovery goals.
  • Engage in outreach activities to facilitate client participation in appropriate treatments and support services.

Compliance and Documentation:

  • Maintain detailed and accurate records for all assigned clients, including service delivery, care plan progress, and crisis planning.
  • Ensure adherence to HCBS and certification requirements by completing required training.

Community-Based Support:

  • Provide services in various community settings, including client homes, ensuring accessibility and convenience.
  • Offer transportation services as appropriate to support client needs.

Additional Responsibilities:

  • Report client concerns to the supervisor and other treatment team members as needed.
  • Participate in an on-call rotation to provide crisis support.
  • Perform other duties as assigned.
