Qualifications

Responsibilities

  • Manages and monitors staffing ratios, transportation, and day-to-day operations of Community Habilitation and Respite services, ensuring OPWDD compliance.
  • Develops individualized, skill-based goals for each person supported and ensures active participation in meaningful community-based work experiences.
  • Participates in interview processes, hiring decisions, and ensures staff adherence to policies and procedures; reviews timesheets for accuracy.
  • Maintains ongoing communication with families, care coordinators, and stakeholders regarding goals, progress, and opportunities.
  • Coordinates and expands partnerships with local and public entities in the community.
  • Ensures program staff stay up to date on all trainings and evaluates program outcomes for continuous improvement.
  • Leads staff training on topics like community integration, dignity of risk, informed choice, and person-centered planning; oversees documentation.

Job description

  • Manages and monitors staffing ratios, transportation, day-to-day operations of Community Habilitation, Children’s Respite, Adult Respite, ensuring compliance with OPWDD guidelines and internal protocols.
  • Oversees the development individualized, skill-based goals for each person supported and ensure active

participation in meaningful community-based work experiences.

  • Participates in interview process, hiring decisions, and ensuring all staff is held accountable to all policies and

procedures. Reviews and approves timesheets to ensure accuracy.

  • Maintains ongoing communication with families, care coordinators, and other stakeholders regarding goals,

progress, and opportunities.

  • Coordinates and expands partnerships with local and public entities in the community.
  • Ensures all program staff are up to date on all trainings.
  • Evaluates program outcomes and implement strategies for continuous improvement.
  • Participates and develops person centered place.
  • Provides leadership, guidance, and performance oversight for staff
  • Schedules and facilitates staff trainings on community integration, dignity of risk, informed choice, learned helplessness, person-centered planning, and documentation.
  • Maintains Program budget and ensures it is being followed to utilize resources effectively and efficiently.
  • Participates in screening process and ensure they are completed within 5 business days of receiving referral.
  • Represents the CFSS at agency meetings, community events, and external forums.
  • Tracks and report on program data, incidents, and achievements.
  • Ensures direct billing is completed.
  • Remains current of education opportunities and community outreach for increasing census
