General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.
Essential Duties
- Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.
- Use only authorized abbreviations established by this facility when recording information.
- Report all changes in the resident's condition to the Nurse Supervisor / Charge Nurse as soon as practical.
- Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
- Report all accidents and incidents you observe on the shift that they occur.
- Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
- Report any known or suspected unauthorized attempt to access the facility's information system.
- Perform only those nursing care procedures that you have been trained to do.
- Ensure that the resident's room is ready for receiving the resident (e.g., bed made, name tags up, admission kit available).
- Greet residents and escort them to their room.
- Introduce residents to their roommates, other residents, and personnel as appropriate.
- Make residents comfortable.
- Inventory and mark the resident's personal possessions as instructed.
- Store resident's clothing.
- Assist residents with packing their personal possessions when transferring or discharging.
- Transport residents to new rooms or to the receiving area.
- Assist with loading/unloading residents to/from vehicles as necessary.
- Create and maintain a warm, personal, and positive environment throughout the unit and shift.
- Meet with your shift's nursing personnel regularly to identify and address problems and improve services.
- Report all complaints and grievances made by residents.
- Participate in and receive nursing reports as instructed.
- Follow policies concerning exposure to blood/body fluids.
- Make beds as instructed and put extra covers on request.
- Measure and record temperatures, pulses, and respirations (TPRs), and weigh and measure residents as instructed.
- Ensure residents unable to call for help are checked frequently.
- Answer resident calls promptly.
- Check residents routinely to ensure their personal care needs are met.
- Assist residents with identifying food arrangements and inform them of food on their tray, location, temperature, etc.
- Record food/fluid intake and report changes in eating habits.
- Keep water pitchers clean, filled, and within reach.
- Perform after-meal care, including removing trays and cleaning residents' hands, face, and clothing.
- Participate in and receive nursing reports upon reporting for duty.
- Perform tasks according to established policies and supervisor instructions.
- Follow work assignments and schedules.
- Cooperate with inter-departmental personnel to maintain nursing services.
- Notify the facility if late or absent.
- Report occupational exposures and assist with resident functions such as dental care, bathing, dressing, etc.
- Assist with lifting, turning, moving, and transporting residents.
- Perform restorative procedures as instructed.
- Assist in preparing residents for medical tests.
- Check residents routinely to meet their personal care needs and observe/report any skin or injury issues.
- Report residents leaving or missing from the facility immediately.
- Use only trained equipment and report defective equipment.
- Maintain confidentiality of all resident care information and report any unauthorized disclosures or abuse.
Supervisory Requirements
This position has no supervisory responsibilities.
Qualification
Education and/or Experience: Must possess at least a 10th-grade education.
Language Skills: Ability to read, analyze, interpret, write reports, and present information effectively.
Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions.
Reasoning Ability: Ability to solve practical problems and interpret instructions.
Certificates, Licenses, Registrations
Must be a licensed Certified Nursing Assistant according to state laws. CPR certification preferred.
Physical Demands
Must occasionally lift/move up to 25 pounds. Prolonged computer use. Regularly sit, stand, walk, talk, read, or hear. May assist in evacuations.
