Job summary

Clover Health Services in Honokaa, Hawaii seeks a Therapist, Physical Therapist to evaluate patients, design individualized treatment plans, and administer therapeutic exercises, manual therapy, and neuromuscular re-education to restore mobility and function.

You will supervise PTAs, coordinate with physicians and care teams, document evaluations and progress, and discharge patients with maintenance and self-management plans for long-term recovery.

Qualifications

• Evaluates patients' physical condition and movement limitations.
• Develops and implements individualized treatment plans.
• Administers therapeutic exercises, manual therapy, modalities, and neuromuscular re-education.
• Treats patients recovering from injury, surgery, or chronic musculoskeletal conditions.
• Monitors progress, adjusts interventions, and records outcomes.
• Educates patients on injury prevention, ergonomics, and home exercise programs.
• Documents evaluations, treatments, progress notes, and discharges.
• Supervises PTAs and therapy aides and communicates with physicians and care teams.
