Responsibilities

• Directly work with incarcerated populations through the detention center.
• Participate in the multidisciplinary data focus group to identify community resources for re-entry.
• Link clients to housing, treatment and social services to promote recovery.
• Use personal stories to teach and mentor clients and families.
• Encourage client and family voice during plan development and implementation.
• Attend team meetings to support clients and families.
• Help providers understand the importance of family involvement in services.
• Explain plans to clients and families and empower self-advocacy.
• Complete duties within scheduled time frames.
