Hyacinth Foundation in Paterson, NJ seeks an HRC Case Manager to support harm reduction outreach and client referrals under the supervision of the Statewide Harm Reduction Manager. You will provide case management, coordinate with community partners, and maintain confidential records.

Responsibilities include 70% client services, 15% reporting, 10% advocacy, and 5% training. Flexibility to work some nights and weekends is required; bilingual English/Spanish is a plus.

Qualifikationen

• Bachelor’s degree or equivalent experience.
• 1–3 years in Harm Reduction or social services.
• Active listening and trauma-informed approach.
• Bilingual English/Spanish a plus.

Aufgaben

• 70% Client Services: conduct outreach, recruit clients, provide case management, refer to services, follow-up on referrals.
• 15% Reporting/Monitoring: maintain confidential records, prepare monthly reports, update reporting system.
• 10% Hyacinth representation/advocacy: attend meetings, build partnerships, represent the agency at events, ensure confidentiality.
• 5% Training and certifications: attend opioid and harm reduction meetings.
